c5 z06 clutch will not disengage

2001 c5 zo6 will not go into forward gears -- only reverse has abs, traction control and handling lights on - lights have been on for months and car runs fine - backed it out of my garage and will only go in reverse -- when I put it in any forward gears and let the clutch out it dies
